	Mark sheet: Two sets of Xerox copies of Mark sheets (duly attested) of qualifying / last attended examination(Higher Secondary(10+2), I, II Year of Graduation, Post Graduation)
	Photograph : Two recent passport size photographs.
	Original Transfer Certificate and Character Certificate.
	Students from M. P. Open Board also have to take eligibility and have to submit migration certificate
